St. Steel crowns

An SSC is the preferred restoration for: (1) primary molars with extensive multi-surface caries involving three or more surfaces; (2) primary molars following pulpotomy or pulpectomy where the structural integrity is compromised; (3) primary molars with developmental defects (hypoplasia, amelogenesis imperfecta); (4) children with high caries risk requiring maximum-durability restorations; (5) behavioural or special needs patients where the restoration must survive until natural exfoliation without requiring retreatment. For isolated small class II caries in cooperative children, composite or GIC restorations may be first choice.

Measure the mesiodistal width of the tooth using an orthodontic gauge or by reading the width from a periapical radiograph with a magnification correction factor. Select the SSC size that seats to full cervical contact without requiring excessive force. Clinically, select the smallest crown that fully seats to the gingival margin without gaps — then try the next size up and compare. If both sizes appear similar, choose the smaller size and crimp the margin to adapt. A crown that requires more than 5–10 Ncm seating force is likely undersized and will fracture the crimped margin or cause interproximal tooth movement.

Stainless steel crowns are metal-colored (silver appearance), highly durable (5+ years primary molar service life), inexpensive (74 EGP per crown), and require minimal preparation. Zirconia pediatric crowns (e.g. NuSmile, EZ Crown) are tooth-colored (matching primary dentition), but require more preparation, are 3–5 times more expensive, have lower fracture toughness than SSC, and require a strip crown or digital impression workflow for sizing. In Egyptian paediatric practices, SSC remains the standard of care for posterior primary molars due to its clinical evidence base, low cost, and ease of placement in less cooperative children.

AISI 304 stainless steel SSCs contain 8–12% nickel by weight, but the nickel release rate from intact, corrosion-resistant SSC surfaces in the oral environment is very low (<0.5 µg/cm²/week), well below the European threshold for nickel allergy elicitation. Documented nickel contact dermatitis from SSCs is rare in the dental literature. If a child has a known systemic nickel allergy with dermatological manifestations, discuss with the paediatric physician before placing SSC and consider zirconia pediatric crowns as an alternative for anterior teeth where aesthetics are a concern.
